Mucking about with hormones is not a good idea unless you have medical support; it is not risk free, especially at the age of 54 by which time your body might well be more predisposed to blood clots, liver damage etc. Dont be under misapprehension - these can kill you. I had to stop my treatment a year ago for one month, when according to my blood test results I might have had a tumour as a result of the treatment.
Your main treatments are going to be female hormones and testosterone inhibitors. Female hormones alone may prompt some growth of breast tissue. In combination with the testosterone inhibitors this growth will be more efficient. However, its very common to find that even with heavy prolonged dosage, breast development is disappointing. It will be real breast tissue though - exactly the same as a natural female's and functional.
If you wish to preserve your erection and ejaculation, you will not be able to take the higher doses, with corresponding reduced efficacy in breast development. What constitutes a limit on dose to preserve your function will vary from person to person. Higher doses mean increased health risks. In addition, prolonged treatment will not only effect changes in the breast area but also lead to widespread body changes mainly centred around the redistribution of fat to produce a female shape. For someone like me, these effects are great of course, but may not be welcome to you.
You can obtain hormonal treatments over the internet. But I would strongly suggest that you get medical backup and regular blood tests whatever you do. And just as important to note is that one never knows what such hormones bought over the internet truly are - they could be what they are meant to be, they could be vitamin C tablets and worst of all they could be very dangerous.
