The only pain i have ever experienced was from over-stretching. Avoid the temptation to take on too much too fast. There is a lot of soft skin down there, and altho it may not look it, it can tear. Use caution. And always play as cleanly as possible. i have been lucky to have not gotten any infections, but the Domme i played with had medical training, and was a stickler for as close to sterile conditions as She could get (and still have fun). The longest i experienced any pain from stretching was maybe 20 hours, but usually less than two or three. Do also take it slow when removing a cath... not a place you want "rope burn".
