When a person has a test called a barium enema, once the barium contrast fluid has been expelled, they often fill the colon with air to dilate the bowel loops so that the x-ray will have better detail and abnormalities can be better spotted. This is done during flouroscopic x-ray, where the radiologist can see the amount of distention and control it. The risk is, if this is done in a play setting, is that too much air could be pumped in. Worst case scenario, if the bowel is over pumped, it could perforate, allowing air and feces to leak into the abdominal cavity causing peritonitis, requiring surgery, possibly causing death. You'd hope that someone would not over inflate, would stop if the subject told them pain was occuring, and that the subject would not be so high on endorphins that the danger signs did not register, but that can't be counted on. People particularly at risk are people who have diverticulosis, which is like little weak places on the colon that pooch outward like an innertube that it about to blow. Since many people don't know they have this condition, there is another risk. I can't speak for everyone; some may have had wonderful experiences with this, but I tend to REALLY err on the side of caution when it involves possible trauma to the intestines and rectum.
