My vote is photoshopped.

While I don't doubt a man would actually attempt this, I believe the tissue in question would have torn immediately (not to mention the testicles completely crushed.)  Maybe one of our medical professionals can shed some light on that though.

The stego layer gives away the edit history. Looks like it's saying that the dangling distance wasn't as far...

I can't do a lot in Photo Shop but I did turn the photo upside down to see if he was kneeling and had a weight hanging. Either way it is a looong stretch.
