So I'm not exactly sure where I discovered this, whether it be here or somewhere else. However, quite some time ago I discovered a "survey" if you will, of questions that Doms and Masters can give their subs. It gives an overall list of everything there is to do in our world of BDSM, and the sub can check what their into and not into.

So, if anyone can help me find this list or even a better one, or something similiar. I would be most appreciative!

I have one, all you have to do is email me on the other side and send me your email addy an I can send one that I made, I am in the process of updating it, again. I have seen a few online, but they were too vague for my tastes; it also have some survey questions as well. The few girls I mentor use that as a basic 'guide.'

Not sure if you are looking for a specific one or not, however, google lists over 100k responses for BDSM checklist.  I'd start there.

I'm working on modifying it for "switches" to have a give/receive field.  When you fill out the form and hit submit it organizes it on a page for you to copy and paste into an email (the actual form comes to my email address [email protected] but I can easily forward to anyone who needs it because they forgot to copy and paste.

Some public feedback phrased as a question:
So, on these checklists, I see an activity, whether I've done it or not, maybe a column of give/receive/both and then my feelings on it...

What if I were to say that I've never given X, but I've received it and really like it, but what really flips my switches is the idea of giving X more than receiving it... how could that be expressed with a form? Or are you intending this as simply a conversation starter with clarification and explanation later?

Case in point: in my profile on CM, I say that RenFaires and SCA are hard limits for me, with expert knowledge... WD and a few others know what I mean by that. How can that sort of thing be expressed on this checklist?

BTW - it looks good thus far! Very clean code as well. I'm impressed!

I found many of the online checklists lacking as well.  I ended up compiling the three or four most popular into one checklist that included a few upgrades.  First was definitions for all of the activities listed.  Until I did the research, I had no idea what Agalmatophilia (fetish for statues) was and it is on several checklists.  I made some of the preferences a sliding scale (rather than just homo or hetero, pick one).  And added extra columns so you can indicate if you've done it and how you like it as the "giver" and the "receiver".  I have it in .pdf form.  If anyone is interested, send me an e-mail and I'll try to figure out how to send you a copy.  But in the end, a checklist is really just a negotiation tool.  You still need to have a conversation no matter what.
