Impalers are the opposite of humblers.
Mostly extended impalers prevents sit up's which is good.
If you anchor the ankles and tape a magic wand to the shaft between the thighs, the wearers are helplessly fucked in more than one meaning of the word. Unanchored wearers can only drag themselves around, at the obvious risk of carpet burn.
Impalers have an obvious flaw, in that if the knees buckle and gravity gets full reign, impalers impale medieval style, which is really bad. Even rounded plastic standing still as the body around it descends, can probably open a falling cervix or intestine at both ends.
If you do impale someone chests via their pelvis, don't remove the pole. Leave the pole in on the way to the operating theatre, as it should cork the holes it's torn and slow their death.
I've never considered doing this former medieval execution method vertically for this reason!!
Horizontal impaling reduces the orgasmic/subspace/fainting/sneezing forces involved, but even then, adding a horizontal crosspiece to prevent over insertion is only common sense.

Male humblers work by anchoring the scrotum to either the back of the thigh or the ankles. Either way, straightening the legs results in pain which should prevent the obvious next step of traumatic partial testicular removal.
Woman lack the obvious party bag to anchor around. Clamps to the labia have to be very finely judged, not to crush, not to slip off when tugged lightly and not to not slip off when tugged HARD!!.
Piercing's look great, but the PSI load of a sneeze on that thin steel is risky.
So cramping legs that can lift a 100 pound body, spasming against a 1/250th of an cubic inch paperclip piece of wire, is around 25000 PSI of pressure on the pierced hole. At that point the only question is whether the labia gets split cleanly or whether a chunk gets ripped off.

The simplest safest way for women, is to tie the ankles to the thighs and then the thighs to some nice chest shibari. It will keep them wide open, low and barely mobile in comfort. Plus the wide soft rope should spread the load of any movement they try without causing damage. Kneepads should be considered. "Monica" stick on name badges are riskier
Leather straps can be substituted for rope
For safe "fuck as you move" effect, a soft rubber double ended dong with one end inserted and the other tied to the ankles ropes, should do the job.
