Some don't care if they ever come, some can come if you look at them hard, and some can't come without direct clitoral stimulation. It varies from girl to girl, ya know? I love your answer, LotusSong, because for me it is so true. You can pound 90 seconds, 90 minutes, or 90 hours and unless the clitoral stimulation is done and done correctly, orgasm will never occur. Contrary to what many people - mostly men - would have a woman believe, there is nothing "wrong" with that. If you, unlike some guys, actually even care about pleasing your partner via orgasm, you need to experiment and find out what brings her to it. Don't think one approach is going to work for all. Same thing with the "denial" concept. While I find that hot sometimes, if it were to continue indefinitely, that would most surely not be the best approach.
