As a wearer of a CB3000 there are some things that I might suggest to aid in comfort and long term care. First, Ring and spacer size.... The 3000 has several sizes of both to help with custom fitting for you. Yes the scrotum will stretch and feel uncomfortable but the ring diameter and spacer thickness you use should not (as was previously said) cut off blood circulation. After 10 minutes of wear, check to see if the "boys" are still warm and don't have a purple look to them. If cold or purple increase ring size. The CB3000's ring is hinged at the bottom using a rivet. I found that it was more comfortable to wear the ring with the rivet head (rounded end) away from my body to provide a smoother surface for the scrotum. Second, Self care... I am released each day to shower and shave. Rarely has shaving caused an issue for me with an occasional ingrown hair and being hairless avoids getting hair pinched in the device. During this time I check for abrasions and clean both myself and the device. When putting it back on, I use baby oil to lubricate the device and myself. This can give a little bit of slippage and keep the skin from pinching. If you don't like the smell of baby oil, mineral oil is the same stuff without the perfume. For those who have a "no release" protocol, Shower with some cotton swabs available to stick through the openings and clean that way. Follow it up when drying off with cotton swabs and the oil. Thirdly, Erections... Yes you can still get them. For me it happens most often in the early morning when hormones seem to be higher. Yes there is discomfort when that happens. In my case it is not just girth of my penis being restricted by the tube but as the erection tries to happen, length pushes the tube away from my body and so my scrotum is stretched at that time as well. Hope this is helpful
