The thing you have to remember was that prior to 1970 or so, there weren't any open organizations for those with consensual BDSM interests. Mostly we existed only as couples or small groups. Thus there wasn't much chance to "build a culture."
I'm sure there were ceremonies, but they were a lot like the toys... we made them up as we went along.
What has been called The Old Guard (post 1945) was heavy into ceremonies, drawing as it did from the military tradition, but since it was exclusively gay, someone like myself rarely got a chance even to observe let alone participate.
Earlier legendary groups like The Hellfire Club of over a century ago, also had its ceremonies but wasn't much like our modern view since according to most historians "consent" was purchased rather than obtained freely and it seemed to have vanished rather than being a precursor for present day activities.
Our terminology was very limited. "Slave", "Master", "Sadist" and "Masochist" was pretty much all we had.
The 90's was pretty much when the explosion of information sharing took off. My book was only the second non-fiction treatment of this for the heterosexual scene.
