Unless your development architecture supports incremental roll-outs through the live server farm, you'll hit a point where you need to bring shit down once in a while to make MAJOR changes. The problems creep in when you don't have sufficient time/resources for a full-on Dress Rehearsal of the switches, then something doesn't work right, then it's a rollback ( did you do a Dress Rehearsal for the rollback ) ? Tiering shit right helps a lot. e.g.: You're Web-Front Ends can detect when the database has gone down, and fall back to Static Content.
