A hole through a dildo would seem to be the answer but its not so easy to accomplish.  
When you drill through elastomeric material...ie silicon...you don't get a clean hole unless you freeze the material prior to machining and I'm not even sure that a silicon dildo can be frozen short of dipping it in liquid nitrogen.

Here's what I'd do....

1. Get one of those "make your own dildo kits"
2. Get a dildo of whatever size you want to use as the "pattern"
3. Make the dildo according to the directions in the kit but with a piece of silicon tubing in the center to create the breathing hole.
