I know i have seen a checklist of sorts, that typically a sub would fill out to detail their interests, experiences, limits, etc. i stumbled across it on the web before, but can't find it for the life of me anymore.

does anybody know what i'm talking about and possibly have the link to it?

Google BDSM checklists.  You'll find many to choose from.  Some allow you to print them out and have columns that you can fill out and share with your partner.

i made my own... it's a compilation from all the various websites i could find listing fetishes and interests, along with a collection of questions i commonly ask people, i see asked frequently, or i was just generally curious about. When i want to know someone, i want to know what's inside of them, as well as what goes on in their fantasy world. my list is unfortunately (fortunately?) very long, and thankfully i no longer need it! But if you'd like to see it, i may be able to undust it off and pass it on.
