Is " sensual trampling" legal? ( and what if you pay to get it?)
What about safety issues? ( blood, etc.)

Legality really depends on your location. And I would expect there to be significant safety issues (injury to kidneys comes to mind, but it would depend on the weight of the person, placement of feet, type of shoes, etc.), but I don't have practical experience, so will defer to those who do.
