okay...Ladies if You will?
do ANY of You Ladies here have ANY form letters of questions to keep in a file on Your boys(or girls for some of You) that You can tuck away for use later? medical questions allergies...food allergies..full name/d.o.b. etc and including likes/dislikes..fav movies/books and whatever else You Ladies deem to be important about Your underlings...

i tried castle realm but found nothing there..
i am still going to do a search.

and

while i am here and have-the floor--
are there/is there/ such a thing as form letter in questions dominants SHOULD be asking to weed out the wannabes...and those sincere?

why do i ask this?

coz Mistress Laura asked me to.
She is still hunting for a couple more boys down-the-line and wants to start a file on ME too while She is at it..

ANY--ALL constructive...help is appreciated.......

My two most frequently read BDSM websites are:

Ambrosio's BDSM Site

for legal updates and a plethora of other links that relate to Law and BDSM.

Click on the "Dating and Communication" link on the left hand menu. It takes you to a selection of readings, one of which is "Negotiation", and another is "Contracts and Collaring ceremonies" where you will find a selection of questionnaires and contracts to help get you started.

And

The Society of Janus

(where you can find links to regional organizations that will most likely have links to the BDSM Questionnaires and BDSM Contracts you're looking for to get ideas for your own:

I use an extensive questionnaire that consists of approx 15 pages of specific questions. Mine includes questions about medical history, diabetes, heart conditions, etc. along with the usual survey about a petitioner's likes, dislikes, fantasies, and experiences.

I first drafted Mine using something very similar to Jay Wiseman's that is available for reading at

Yes I too drafted my own form using Jay Wisemans check list as a starter.
I also browsed a lot of web sites and thought very hard about the sort of questions I wanted answered.
The other thing I did was to *test run* it on a couple of good friends.
I believe as a commencement to a session with a new person this is a great thing to do. Reading the questions out loud and making them answer them, and think about the reality of what their answers may bring.
On occassions I have used the form filling out as a session on its own. To give the submissive time to think about what might happen to them in the very near future.

I did this as well.

I also divided the check list into different levels of answers -- experience, desires, neutral, soft limit, and hard limits. One can check off several thing each activity or interaction on the list.

And I learned to never give out my own checklist (the one I've filled out myself) before the potential. I noticed that too often their answers changed to reflect more of what I had down.

Oh, and I also don't hand them the check list to fill out but to look at -- we fill it out together so I can ask questions and take notes as we go. I have the three hour plus negotiation lists before I offer a training contract to anyone.

While I don't have a specific list of questions that I ask, I *will* compile most written communication (emails, IMs), plus take notes on significant information from phone calls, and after getting home from meetings. The email introduction someone initially sends me is always in there. All of that gets printed and put into a file. When I stop seeing someone, the file goes in the shredder.

*Why* do I do this? A couple years ago, I was interviewing multiple subs for servants at a regular tea party I hosted. I had a long questionnaire that they had to fill out, followed by an in person interview if I thought they had merit. I liked having a bit of written info on people on hand. Also, I have a tendency to have a really BAD memory of things like birthdays, anniversaries.
