One of our locals brought it to our munch that evening, and...although I don't get the joke...it seems to apply clearly to WIITWD.

I don't get it either but at least it lead me to the Dilbert strip.

It's a reference to a football huddle.  At the end of the huddle, the quarterback says "BREAK!".  To force something physically is to break it... the opposite is to force something mentally, or to dominate it. 

The surgeon doesn't want to break it, he wants to do the opposite.

Okay, it's seriously a stretch, and I think the cartoonist was sniffing the ink a wee bit too much that day, but that's the best I can come up with.

Not from any RPG Ive ever done, I still stand by my first impression.... we must remember the word dominate appears all over the place irl

The only rpg it could reference is V:tm as far as I see.  In it dominate is the vampiric ability to control other peoples minds to make them do things. Meh.

it's a sporting refference.  im pretty sure in this refference the surgion is trying to pep talk the team into doing a really good surgery.  as in, alright we're gonna go in there and dominate our competition (the offending body part).  the one, two, three... bit is deffinetly sports

My thought was 'Dominate!' should have been 'Dominant!' but... who knows.

I agree that it's probably a sporting reference.  It looks like their hands are all in the center of the huddle, head doc says "Okay 'dominate' on three!"  And we assume he says "One, two, three!"  and they all yell "Dominate" and break. 

Or maybe there's a little bit of an anti-religious reference there.  Instead of praying for divine assistance before operating, they go into a huddle.....?   

Neither is very funny in my opinion, but it's the best I can do.

lol think of it in this manner.

A group of friendly "Bdsmer: get together at a local dungeon.
One of the newer members says,  "I always wanted to scene a medical thingy!" "Does anyone want to show me the ropes?"
5 Dominant personallities all volunteer to help.
Not being able to decide, she says, "Let's make it a team effort!"
In order to play nicely and make sure she was ready for the intial assualt, she made them promise to "Dominate" on a count of  Three. :P
