Well, fasting is hardly needed. (It can be a nice touch, and a fun part of the ritual, if you like, but it's hardly a requirement.)
From the talks I've had with women whom I've done this sort of play with, I found that (physically) they seem to enjoy the sensation of being penetrated, without the worry of accidentally getting pregnant. Secondly, they find that the feeling they get from being filled there tends to give them a different, somewhat muffled pleasure compared to direct vaginal/clitoral stimulation.
For men, the physical attraction is the prostate massage they get, and also the intense feeling of being opened up. However, I think with men, there is a major phychological component to the attraction: they are basically reversing roles, and allowing themselves to be "taken." To males, this is probably the second most submissive act possible. (Cocksucking being a close second.) Males think that they own the copyright on fucking someone... so, when they are in turn the one who is fucked, it plays holy havoc with their psyches... (usually in a good way)
Of course, both sexes probably enjoy it because the anus is packed with nerves, and the sensation is so intense that sometimes it will cause orgasm in women, and a "short circuit" in males. (Where the feeling is so intense that they really can't process it, and an erection at the same time... which is why some guys go limp when being fucked.)
There are plenty of other reasons, of course. Many of them are as personal and specific as the individuals involved. But I feel fairly safe in saying these are probably some of the major ones.
