As far as your balls hurting, you two might want to spend a bit more time finding the right fit before you start long term. Start with a large ring and see if you can go a few days and nights. Then go with a smaller ring and repeat. Eventually, you try a ring just a bit to small to be comfortable. Go back up one ring size. Also experiment with the spacer. Once you find a good fit, expect to go back to the smaller ring after a couple weeks, as the scrotum may stretch out some. One thing I found key to long term use is the application of A+D Ointment (http://www.myadbaby.com/products.html) to one's penis before locking it up. Tried both water and silicone based lubes at first. But always ended up getting dry and chapped skin on my penis. Being uncut, the foreskin would be a wreck after a couple week session. And that sucked so bad. Finally given some freedom and the foreskin would be too sore and cracked to be able to have fun. But with regular application of the A+D after cleaning and re-locking, I would be good to go for almost a week at a time. Also good to apply some around the ring after each shower, just to make sure the ring can move easily during the day. And that brings me to my last point. I don't know about others, but DO NOT go 30 days without a complete cleaning. At least once a week, have her unlock you before you shower and clean yourself completely. Chastity play is cool and fun, but not to the point of breaking the equipment it is protecting. You can be trusted to be good in the shower, right? 30 days is a long time. My advice would be to work up to it. First one week, then release. Then two weeks, etc. Each week brings with it a new set of emotions and best to give yourself to learn how to process them.
