There's no law book on how things are to be done.

So mainly, proper rules and etiquette are those that you and your partner decide to have between yourselves.

Emily Post?  Good manners are pretty much universal unless, as said, you've chosen separate rules for WITHIN your own relationship, and then still it's just in your relationship.

The rules depend on who is holding the leash... simple

Beyond the fact that there are no universal rules...your search engine would work better if you spelled it correctly: etiquette.

what everyone else said - you and yours make it up according to your relationship. one book or rules of etiquette cannot universally and equally define every BDSM relationship.

I think it is something you have to discuss between you and then put in writing? Differrent for each individual I would think? Jerry

Yep, so now you get to educate your partner on how the lifestyle works.  Wonderful isn't it?

What she said. Anything else will change from person to person.

One might suggest a dictionary would be a fine place to start...

To follow what everyone else has said... the only universal proper etiquette is to be polite.
Rules come from the one you are with, under consideration or owned by. Knowing how I expect my 2 boys to act, different rules for each, I know that what works for one doesnt for the other.
