I built one of those for an online buddy one time. It was pretty simple, and I had to try it out myself first to make sure I got it right.
Yes, it is very effective. Standing in anything but a squat was impossible, and you didn't really want to do even that. Kneeling was doable but still hard on the balls, and sitting in a chair was all but impossible. The only really comfortable position was on all fours, and crawling around with one of those on is pretty hard to do.
As for how tight...The hole your sack fits through has to be small enough not to let your balls pass through, but not so small that you cut the blood off. This was a couple of years ago, but I think I made his with a 1 inch diameter hole, nicely smoothed off of course.
Considering the postion it forces you into, the Humbler is a good name.

I got mine years ago from Hanson Paddle....finished oak, with a built in lock and attachments for a TENS unit.

Tehe!

I Purchased a humbler several years ago. It had a curved area that was tapered about 1 1/2 inches long by about 3/8 inches high. instead of a hole for the ball sack. I samded this down to about three inches by 1/2 inch. It works much better now The two wooden piecec can be easily brought tightly together by the wing nuts withou pinching the skin on the side of the scrotum.

You install it from the front and then push it between the legsto keep you bent over slightly with the one I have. I replaced the stove bolts, and wing nuts on the two ends with eye bolts that thread into Tee nuts that are installed in the wood at each end. This allows wrist cuffs to be attached to the eye bolts so that the device can not go back between the legs. This also means that the eye bolts can not be turned to loosen the device.

My mistress put me in this a a play party and I wore it for four hours.
