Being a huge fan of CBT myself, here's a list of some of the types of play that I enjoy.
Learn how to whip the ends of rope. The same technique can be applied to the scrotum between the body and the test tickles. Parachute cord is good, tiny hemp rope is better. If you go tight enough to turn them purple, you should limit the time they're tied up to fifteen or twenty minutes. If they go numb, it's time to untie them.
Sounds and catheters. Ureathral insertion isn't really painful if you're doing it right. But the idea of it really squiks a lot of guys. If you get into this sort of play, cleanliness and proper technique are crutial. The best approach is to learn from somebody with a lot of experience or a health care professional.
If you have a farm supply store near by, you might want to score an elastrator setup. There's a plier looking gizmo that stretches very small latex bands so that sheep testes can be pulled through. Then let off the handles, and roll the band off the pins one by one. It's best to lube up the scrotum before application. Get a pair of cat claw scissors to cut off the bands. The cat claw scissors will have rough edges, smooth them with fine emory cloth before using them. An elastrator band will turn the balls a dark shade of purple. 15 minutes is the longest you should ever leave one on. Elastrator setups can also be had from some adult toy stores for about three times the price. Also, the most intense part of an elastrator scene is when you cut off the band, even after only a couple of minutes, you'll get a good scream out of it.
Electrical toys are wonderful for CBT. A TENS unit either with pads or purpose build CBT accessories can be quite fun. My favorite is a stun gun. 200,000 to 300,000 volts is best. High dollar stun guns are much stronger, buy a cheapo one for SM play. Swap meets and gun shows are the best sources.
I really like being kneed in the groin. Not too gawd awful hard, if you start out somewhat gently, then build up in intensity, you'll have a lot more fun. Having the guy sit on a low chair or stairs then stepping on his balls is quite nice also. Some guys really enjoy having their testicles held in one hand then smacked or punched with the other hand. Or just grab hold of his boys and squeeze or pull.
If you're going to do any purcussion on the cock itself, remember that it can't take as much impact when erect. Never do anything that bends or squishes flat an erect penis. If you rupture the erectile tissue, you will have broken your toy and odds of it ever working properly again are very slim.
Also, sharp pokey things are lots of fun on the boy parts. Unless you have real live piercing training, don't actually penetrate the skin. This is where a little mind fuck can be a lot of fun. Lay out your toys, include a sharp knife or scalpel and some sort of needle or a Whartenburg Wheel, then blindfold your boy. The edge of a credit card feels remarkably like a sharp blade, a bamboo skewer feels like a needle or the tip of a knife, and so long as you don't apply pressure to it, the Whartenburg Wheel is fine for genital use.
Don't be afraid to improvise, just keep it within reason. I have a beaver trap that I just love GENTLY closed over my balls. I've seen rat traps used on balls, and mouse traps on cocks. Just don't actually snap a trap closed on anybody you want to play with again.
And I'd be remiss if I didn't mention wax. Jesus candles, white utility candles, and tea lights are good. Don't let the tea lights burn long enough to melt the whole thing. Drip it as it melts. Try any candle on the inside of your forearm before dripping onto anybody. Some candles melt hotter than others. And since you went so far as to buy candles, you might as well have a bowl of ice handy.
I know, I've written a novel here, but I'm just about done. Always start out a new type of play gently and see how he handles it. Not every guy enjoys the same sensations. For instance, you can tie my balls up tight enough to turn them cherry red and I'm having a good time, but wack them even gently with something hard and the scene is over. Most likely, some of the play I and others have mentioned will be a blast, while some other things will really suck. You will probably find certain sensations that send your boy flying, and others that'll have him pulling up his pants. I hope my ramblings are helpful.
