I can beat someone physically all day long.  I can check his/her body for scars from prior injuries or see wounds and avoid those areas.  The same isn't true on the mental/emotional plane.  Unless I put the work into the talking, investigating, researching, and discussing, how in the hell do I know that something I might use might trigger a horrible childhood memory of when someone was gang raped when they were less than a consenting adult?  I don't.  That makes it dangerous for Me and potentially harmful to the person I'm playing with.
