I want to be able to carry smaller items of BDSM gear (like a little leather switch and a pair of cuffs) in a shoulder-holster type of arrangement. Why? Because it would look hot, be convenient, and I could wear a jacket over it and at some point slip open the jacket and reveal what was under it, like people do with their guns in gangster movies, and my sub would be like "oh noez!" and it would be pretty great.

Is this possible? Would I want to start with an actual shoulder holster for guns (it might be hard to find one in my size; I'm a tiny person), or start from scratch and make myself one?

I would talk to a local leather worker and see what they could offer you. There will be a few of them at Purgatory in Charlotte on Feb 14th and/or April 18th (we are changing April to the third Sat for some reason).

You could also get a gun holster and have it altered. This may give you a more 'professinoal' look, but will also cost considerably more (I would gues). Tiny girls need protection too, so I'm sure you could get a manufactured holster in your size.

If you make it to Purgatory, I'd be glad to introduce you to one of the leather workers and encourage him to be good to you... I'm easy to find... I'm the guy in the middle of the big room tying up girls ;-)

You should check out shoulder holster companies online, since they do make them where you start with a standard shoulder harness and add the accessory hangers to it. They do make them with with hand cuff holders and various clip and billy club holders. and you just snap the holder desired to the shoulder harness.  I don't know of any names off hand but i am sure online law enforcement accessory shops have them .

Shoulder holsters are expensive, because they're somewhat tricky to fit, and need to be made to a fairly high standard so that people can wear them comfortably all day long. It would be a good place to start, particularly for handcuffs, but you'll be looking at at least $100.

Another thing you might consider is just modifying the inside of a jacket, or especially a long coat... you could whip open your trenchcoat and show off a whole row of floggers hanging from the lining.

Then there's the spine sheath for the giant two-handed paddle that you can draw over your shoulder like a Claymore...

I like your idea, but don't have any info concerning the best way to make a holster such as you need.  I do want to give a word of caution to check your local handcuff laws.  Some states view handcuffs very similarly to a weapon.  In Florida even carrying a concealed handcuff KEY, alone, is a felony.  Better safe than sorry.

I suggest going to a Army/Navy surplus store and checking out the gear in person so you can try it on right there.  You can find amo belts, knives and paint ball gun supplies among other items.  Plus their items are usually less expensive and very sturdy.

Does it matter if it's leather or nylon?   Most shoulder rigs should have a lot of adjustment room.  You can cut or remove the holster and replace it with a panel that has a snap clip.  Or keep the large size holster,, say for a 1911 or full size Glock, and stuff a flogger  in it.  A cuff case can slide onto the off side.  Galco makes a dedicated cuff case for their shoulder rigs.

But I would buy the cuff case and then make the rest.

But also concider the design, a 'spider' design  consisting mostly of straps, starting with the X  between the shoulder blades or a "H" design that consists of a single strap between the vertical straps that go over the shoulders.

I like the "H" style as it seems a little more stable (I have carried a Sig Sauer in one for a while)  I think this design would be more adaptable to your needs, with a couple of cuff cases on one side and a panel with snap clips or laces or hooks or whatever you need for your toys.
