Wow - I am surprised that I am having such difficulting in finding what I am looking for, hopefully someone here can help :-)

I have thick calves and thighs, and am looking for a pair of thigh high boots.  I would be fine with having them custom made, but can't find anyone who can do this!  Is there anyone here who might have some suggestions or know someone?

No help on thigh high, but if you could settle for knee high riding boots then Ariat makes them in three leg lengths plus a wide calf. And with a hidden zipper in the back. I suggest getting fitted at a tack shop instead of just ordering.

Excellent ! - I'm glad you found the info helpful !  I've ordered from those folks before - their shipping dept is on the ball and product quality is always impressive.
The founders of EHH used to have a page on their site that detailed how the company began. Frustrated with finding so few sources for sexy shoes for their girlfriends, they started their own company to address the supply problem!

High heels, heigh ho. Sexy boots with flat to a max of one inch heels would be a winner in my book! You'll notice Pleaser may make every size on teh planet but their lowest heels are 2/5 inches.

skyscraper in chicago  does custom work for all kinds of shoes,boots,corsets and are reasonably priced.

James Gottselig, an artisan who trained with Richard Burden for Bald Mountain Moccasins now has his own company.   (Mr. Burden is now retired)

I was just in Macys and saw these Jessica Simpson boots - they are hawt !!!

The store only had them in size 10 so I didnt get to try them on (couldnt even fake it since I'm a 6.5/7)

Thanks for everyone's suggestions!  Alot of the commercially available thigh high boots aren't great for me as my legs are thicker, but I did have some luck with skyscraper heels!  Thank you for all the info, I will follow up some of the other newer posts and will update if I find any others that can customize to larger thighs.
