Current diagnostic standard for sexual sadism: http://www.behavenet.com/capsules/disorders/sexsadismTR.htm

and sexual masochism: http://www.behavenet.com/capsules/disorders/sexmasochism.htm

Part B of both diagnoses, "the person has acted on these urges with a nonconsenting person, or the sexual urges or fantasies cause marked distress or interpersonal difficulty" and "the fantasies, sexual urges, or behaviors cause clinically significant distress or impairment in social, occupational, or other important areas of functioning" makes it harder to diagnose those who practice consensual BDSM as mentally ill, although probably not impossible.

"the sexual urges or fantasies cause marked distress or interpersonal difficulty." this can be in the eyes of the doctor OR (and most likely with many doctors) in the eyes of the patient. Happily there are sites like this one that lessen the sense of interpersonal difficulty to a degree.

Basically, if you don't allow it to make you crazy, then you're not crazy.

Or if you think you are crazy because you want what you want, then you are. It goes both ways.  And to me, that is one of the biggest reasons for outreach programs like munches and on campus groups and things, so people don't have to drive themselves crazy trying to want what they don't want.
