I was thinking, what if you used a belt around someone a la this:

Safe? Probably, assuming that you didn't crank it super tight and leave it on for overnight. However, I don't know if something would stay wrapped like that, no matter how tight it was.

How about using saran wrap and electrical tape?

Something like this. I wanted just a head shot but this was all I could find. But you get the idea of what you could do.

What specifically are you safety concerns? 

Assuming that the belt were tight enough to prevent the mouth from opening, the subject would not be able to clear any GI emesis or respiratory secretions, but this would be no different from any closed mouth mask hood or gag. 

In terms of teeth grating, it would depend on the person but a moldable athletic mouthguard would fix that problem.

From a bottom's perspective, this doesn't seem like it would be much of a restraint.  What is it that you hope to accomplish with it?
