As with all things definitional, YMMV. As I use the term, worship is a form of service, characterized by the expression of devotion as the service is performed.

When worshipping, show your devotion to your Dominant with your words and actions. A backrub is a backrub, a backrub with expression is something else. The feeling transcends the natural physical pleasure and relaxation of the event into something more spiritual.

How deep are your feelings as you bathe or massage him, and is he the unwavering center of your focus? How do you express your commitment to his contentment? How do your hands, lips and breath caress his body?

It could be either or both or neither, depending on the context and your own feelings.

Worship is a fairly personal experience.

This too is how I would define body worship as. Being allowed to kneel at the alter. The alter being Masters body. Praising it, admiring it, worshiping it. It's a gift given to me, and as such, it should be praised and reverred.
