I am curious what this phrase means as I have been seeing it on some profiles I have been looking at lately.
Personal note about this user(visible only to you: does not send a message)?

Click on the little question mark next to it and it says the following:

This feature allows you to
save personal notes about
this user for your own
reference. If you wish to send
a message to this user, use
the 'Send Mail' feature at the
bottom of the page"

So, if you want to keep track of a user's real name, contact information, birthday, etc. you can do it there and it's visible only to you.
