Make your own... It's all good to look at the different checklists. but at the end you should beable to make your own. As a Top, I have mine one for a female bottom and one for a male bottom.
Currently I have six groupings. Sex play, Bondage play, Bondage Toys, Sado-Maso, Fetish, Role-Play
for each item I ask 7 basic questions
Done it - yes or no
Want it - yes or no
Rated Want - Scale 1 to 5
Fear Level - Scale 1 to 5
Humilation level - Scale 1 to 5
Limit Play - Yes or No
Experience - never done it, Rarely, Occassionally, and Frequently done it
the checklist itself is only a basis to for negotiation on what will occur in a play or not occur. I would also be concerned with Health issues and BDSM experiences.
