Are piles hemmorhoids, Peon?  Or fissures?

You do as other posters mentioned, need to drop the concept of "unhealthy" medically. It's not and if you are a medical professional then you should do your own research to discover as others have--that it's a safe experience for those who eat properly (lots of fiber), prepare properly, anally condition and use plentifuly amount of correct lube.

It's been said by sex educators that a vanilla player's idea of lube is a little dab. You cannot overdo lube in ass play.

What might be more helpful to you is to look inside to find the core idea from childhood that makes you shy away from strap ons. You already know that most dommes won't reject someone as a sub or a play partner over the hard limit on strap ons. And none have rejected you. So we're all wondering--what's your real question here?
BTW nearly all the subs I know who rejected ass play or strapons happened to have medical backgrounds! I think it's possible they've seen the negative side of unsafe sex or accidents which is a minority of overall experience. It might be influencing their perceptions of safety.

It's actually the nails, rather than the strap-on, that's the greater risk.  Even meticulously kept nails can cause tears quite easily.  I won't speak for mal, but that is the very reason why I use gloves and will continue to use them.  If nothing else, it's something of a reassurance to Me.  If I'm not ripping the glove, I'm not ripping the boy.  It's not a perfect theory, but it's working for Me so far.

One other side note here.  There are some types of play that are more likely to squick folks who are in the medical field.  There's nothing wrong with that if you have someone willing to put in the extra time and patience to achieve the goal.  It may take a long time for it to happen or it never will.  For most, those kinds of limits aren't a deal breaker.  Maybe the OP is more focused on those who find that it will.

While you're awaiting a reply, here's my preference:
If he's not allergic, I like relatively tight-fitting disposable latex gloves; too loose and you lose sensation, too tight and your hand can cramp. A bit of cotton ball under your nails or in the tips of the gloves helps cut down on glove breakage if your nails or cuticles snag, and no gel tips or other fake nails unless for S/m.
Gloves also make cleanup a breeze, and just like 'ER' you can roll them off your wrist so they're inside out. If latex a water-based lube is required; everyone has their favourite, but I still like Astroglide.
Roughrider or other ridged condoms can be fun, but may come off.
If he's allergic to latex, I find the polyurethane gloves reduce sensation and don't fit well; the 'one size fits most' seem to be made for male hands. Like condoms, a bit of water-based lubricant inside the glove fingers as well as outside can feel more like 'bareback'.
Fingercots [like roll-down condoms for fingers] are another option, but I find them uncomfortable after just a few minutes.
And after just a few times, watch for the Pavlov reaction as a blindfolded sub hears you 'snap' the glove on! Delicious!
