Actually this is not a fantasy, most dom/dommes may require oral sex daily or at least on demand. Most subs/slaves of either sex either enjoy this or consider it a privilege. I myself do this as often as Master allows (lucky for me he never objects, lol).And enjoy it as much as he does. Suckling is different. There are times when he wants to use my mouth to please himself, and there are times when he allows me to rest in my safe place. Some WOMEN can do that- not just slaves. 

And yes, it's possible for some people to orgasm without direct clitoral stimulation (however, I've never known it to happen when the girls legs were forced apart and absolutely NO sexual stimulation beforehand).
