I think it's sexy as hell, but the woman definitely has to be able to relax and get into it. Vagina's do come in different sizes, something noted as far back as the Kama Sutra, although unlike men, women have a fairly large degree of control over their size - it's really a muscle and like any other muscle, it responds to exercise. Women with exceptionally strong pelvic floor muscles from Kegels can practically rip it off, even after several children, and regular orgasmic muscle spasms themselves help maintain good muscle tone and circulation.
In fact, a lack of sexual intercourse can lead to what is known as "senile vagina" which is really just very poor circulation and muscle tone, atrophy from lack of excercise.
So, from a certain perspective, it's the equivalent of guys pulling freight trains, or hanging from the ceiling by their penises, and the like, i.e., testing the limits of athleticism, kinda like yoga, martial arts for the cootch. You should see those Philippina dancers sometime.
