Mostly, it depends on the design, and whether you're using a leather contraption or a steel one. Leather and water and bathrooms don't usually go well together, but in the case of a steel one, it's actually pretty simple.
The design incorporates a vertical slit in the front shield--this allows for some motion of the urethra relative to the shield and ease-of-flow. As for showering--stainless steel isn't much of a problem there. You'll probably be able to get just enough wiggle room to spray into the belt's major areas, or at the very least get water to flow in that direction. It helps to have a detachable shower head. Dry thoroughly on the outside, and maybe use a hair-dryer near the slit or allow to drip-dry if it's an issue. Be careful to rinse thoroughly, though: excess soap left behind can irritate the skin.
As for the other half of the bathroom issue, it depends on the design--but it's taken care of, you can be sure.
