Meat tenderizers work, generally, by chemical reactions breaking down the various fibers within the meat. Metal rods do the same thing but instead of using chemicals (well, herbs, spices, and whatever), it's the physical hammering that tends to mechanically separate the fibers. I'm thinking the metal rod may be more brutal and painful at first but the pain starts to fade as soon as the pummeling stops. The tenderizer, on the other hand, keeps on working it's way down into the fibers long after it's initially applied, and keeps eating away until all of the chemicals are used up... the pain would keep going, and going, and going. Both, I think, would do a fair amount of physical damage but the tenderizer might be more annoying overall.
