It seems Transocean management had decided to turn off all the safety alarms in the lead up to the explosion. According to testimony from worker to federal investigation.

Yeah, I heard that a while ago, same situation as what happened on Piper Alpha, the alarms were said to be too sensitive and went off for explainable but not necessarily it was thought; hazardous reasons. The result is essential safety equiptment was rended useless because people thought they knew better and wanted some uninterrupted sleep.

If indeed hazardos situation warning devices were in fact turned of on Deep Water Horizon, then that proves something , nothing has been learned since Piper Alpha and every operation in between was a cause of good luck rather than calculated risk.
