PA while I dont doubt that de regulation has fucked things up in many ways. Cutbacks to frontline staff  and longer hours is adding to the fuckups.
truly, this is NOTHING new, mistakes, accidents, nurses, doctors, medical procedures have never been perfect, and never ever will be. no matter where in the world you are.
Minimizing the risks  is the best that can happen.
Oh and live your life knowing that nothing is perfect anywhere in the world.

All medications are poisons if misused. That's why the professionals that prescribe them give careful instructions on how much to take and when to take it.

Considering that most condom failure is user error and all you have to do is pinch the tip and roll down a length of latex... I doubt it.
