I recently got a physical exam from my new internist. I was a little uncomfortable explaining a few old marks from the scene. Has anyone had a similar experience?

Doctors have seen it all.

If you get asked for an explanation, you can say it was a ssex accident.

Are you quite sure that you meant to post this in the Mistress section?  The only time I tend to mark Myself is if I've been using the longer whip and even then, it's not that often.  Floggers, tails, and such don't cause the issue.  If I do a take down scene or plan on doing resistance play where I expect the odd bruise and so on, I just schedule that when I know I don't have appointments for a physical or whatever in the upcoming week.  If I have an unexpected mark as a top, I just say that it was an accident, which it obviously was, since I'm not a bottom.

as stated above most Dr's and medical staff have seen lots so if they ask a question be honest and tell them, otherwise they might start to think its some kind of domestic abuse. i have my Mistress mark on my chest and when asked i tell them its Her mark.

I'd look up the domestic violence laws in your state before telling your doctor everything.  In the state where I live, Alpha could be prosecuted for domestic violence without my permission or testimony, even though I'm fully consenting.  I think that's true in a lot of places.  Yes, doctors have seen it all, but sadly that won't keep them from thinking you're some kind of sick freak (a common line of thought) and reporting marks as signs of abuse.

You need to tell your physician you enjoy rough sex at a time when you carry no marks. If you wait until you have them, and are asked about them, they may assume you are being abused. You go in, sit down, and explain what you like with a smile. What you don't want is to be marked, to have a heart attack or be in an accident and have the ER staff see the marks and call the cops. You do want them to contact your physician (make sure this info is in your wallet along with your allergies) and be told that he/she knows about it and it is consensual.

Not really. I have all my doctors put  a note in my file that I like "very intense" sex and that they may see bruises or marks that look strange to them. I've even told my dentist.
