I am very keen to wear a chasity belt for a mistress but am unsure how it works. Do I have to wear it all the time? Who holds the key? How do I pee and wash? What happens if I get an erection does it hurt?

It all depends on the Mistress. Some use it for training, some for punishment, etc. There are many kinds of devices. If she choses to keep you in there 24/7. It should be taken off at least once a week. Both you and the device need to be inspected for "flaws" open area can be dangerous. I personally would check everyday with a new device.
again depending on the device the amount of pain can be different most are designed to hurt during erection and helps the wearer not to get erections thats are not wanted. (training)

Search for chastity devices you can see pics and they usually explain what they do.

There are lots of threads about chastity devices. Do a search.

BTW, you won't likely be wearing a "chastity belt". Most of the better male chastity devices are not chastity belts. The CBXXX series (I prefer the CB6000), and the Birdlocked chastity devices are very popular. You may want to look into those.

Lastly, the answer to all of your questions is basically the same. It is going to depend on what you and your Domme agree on. There is no generic protocol for wearing a chastity device.
