Selling the open, taster bottle at the Spice Guy booth at a Farmers' Market instead of one of the sealed, stock bottles. Big OOPS. Fortunately, found buyer and exchanged bottles <with lots of blushing on my part.> WORSE - that happened twice last season. But zero times this season. We re-arranged booth - seems to "help."

Placing an on-line order for clothing and accidentally typing in the product code for the blouse, not the skirt ooops! (actually liked it so much after it arrived I kept it and then ordered the skirt as I had originally intended to do)

THE all-time CLASSIC: What do you mean that went out to the whole e-mail list? OOPS! Actually happened to a guy who was cheating on his lover who was - of course - on the e-mail list. Break-up happened as soon as cheater got home.

Speaking of mailing lists, here's one that really happened to a former co-worker of mine...we still tease him about it several years later. The guy had a dentist appointment. He meant to send an e-mail to our manager only to let him know, but somehow sent it out to the entire company! Our manager's boss, who hated the guy already, flew downstairs, red in the face and totally livid. Some people in the London office inquired as to how his dental appointment went. I felt sorry for the guy! oops!
