I think as with many other things part of it depends on who handles your paper work. The person who does your interview can have a huge impact on the process.
I have a good friend in Orlando I've know since the early 90's not long after he came over. His wife was an American. They met in Germany, were married 4 years with a 3 year old daughter before they relocated to the US.
It was a long drawn out nightmare. He was asked by the person doing the interview "How do I know you didn't get married just to move here?".
