"Practicing this kind of extreme sex, everything has to be controlled, but he knew that completely. We don't understand what happened."

Don't understand what happened? Drugs, alcohol, bad judgment, All of the foregoing? Not hard to understand.

I do strongly feel that certain types of play do not mix well with alcohol/drugs. But this incident could have happened even without the alcohol/drugs. Even if you play completely sober, anyone practicing rope bondage should make sure they have taken time to learn safe bondage techniques. There are lots of fun ways to play with rope that would never cause death.

We bandy around the term SSC - but how many truly internalize what this means? Things can only be safe if risks are fully understood and appropriate techniques are learned and precautions are taken; sane implies you are in command of your full senses - again it is hard to imagine drugs/alcohol fitting into that definition - even someone signing a contract under the influence of drugs/alcohol would certainly not be considered competent - what makes people think they can put rope safely around someone's neck when not sober; which brings us to the third, consent - can extreme play of this manner be considered consensual when all the parties are under the influence of drugs/alcohol? Debatable proposition in my mind. And also, does a submissive's consent to this type of play mean that the Dominant is not still required to take due care and caution? I think a true Dominant understands the huge responsibility he/she takes on when someone consents to extreme play of any kind. I think it is reasonable to think that if I consent to rope bondage that I might inadvertently suffer a minor rope burn, but death?? Since when is consenting to extreme play consenting to death?

Japanese sado-masochist technique known as "shibari".
This made me go "o_0" as I've always considered shibari as artistic bondage, not exclusively sado masochistic

Shibari is actually very functional and much ties are aimed at inflicting pain, humiliation, desorientation, etc.  Oriental aesthetics are of course an important aspect of it as well, but to describe shibari as primarily artistic bondage imho completely misses the point.
