In My vision, the wrist would be secured on the outside of the ankles. Stay with Me here. The spreader bar should be connecting the two ankles by either side. That means the ankles have some form of loop around them (leather, metal, cuffs, for example) to secure them. This means that you have plenty of material on the outside of the ankle to add an anchor point. Why not take the wrist and affix them there? The arms would be down, rather than the person supporting themselves on hands or elbows, but for your purpose seems quite effective. Like so: ()_____() Spreader bar attached to ankles with link {-} ()_____(){-} Spreader bar attached to ankles with cuffs [other side of ankle put clip point] {-} ()_____() {-} [other side of ankle put a clip point] [other side of ankle attached to wrist]{-} ()_____() {-} [other side of ankle attached to wrist] So, focus on the middle. You have a spreader bar between the legs, hooked to the ankles. An additional hole punch at 180 degrees should allow you to hook the wrists. Not quite "doggy" style because she is no longer able to support herself by the forearms, but presented for use pretty much the same. Knees up, but face down, ninety degree angle. Were you, perhaps, concerned about her discomfort with the face into the pillow?
