It hurts, but not that bad. They will be quite sore for 4 days to a week, and tender for a week or so after that. I found that an ice pack now and then managed the pain sufficiently. And yes, braless and lose tops for a those first few days. Personally I had no problems whatsoever with any of my piercings <regular ears, 2 labrets, tongue & both nipples>. Go to a good reputable shop and follow their care instructions to the T, also don't hesitate to call them if you have any worries, a good piercer will always be willing to check them and let you know if there is an issue and to give advice. This is a little misleading though, because while the actual piercing of the nipple was the most painful of mine, the tongue hurt the most afterwards and for the longest <about 2 weeks>. If you are interested, I say go for it, they are pretty and they are a lot of fun to play with, however, I did notice a drop in sensitivity in my nipples after being pierced.
