Agreed. If there is a swelling in a testicle, the first thing a doctor would do is an ultrasound. Based on that and a clinical exam, surgery would either be recommended, or not. However, an unexplained swollen testicle would almost alway require surgery - especially if the individual is in his 20s - the prime age for testicular cancer.

And for the record - see a urologist.
