I believe many things done improperly can be unsafe.  There are factors to consider when doing electric play.  Health related issues, and experience are two to start with.  I believe that electric play can be really fun in the right circumstances.

My TENS unit clearly states not to cross wires over the spine.  It has two leads…each split.  One lead and the two ‘ends’ can be on one side of the neck or back ………or whatever…. and the other lead and it’s two ‘ends’ across to the other side.  Just don’t cross the spine with a single (split) end of one of the leads.

Crap… that’s clear as mud!  Does it make sense at ALL???   (I hope)

As with everything in BDSM or even in life in general, it is as safe as the person using it, peresonally I have not heard of equipment malfunction in an electrical device from vibrator to violet wand that has cause any harm to any one so far.

Define safe?
As someone who has had friends get broken coccyx from spankings and a dislocated shoulder from a violent orgasm in bondage and heard of one submissive getting a heart attack from the excitement of cleaning his mistress' bathroom with a toothbruch, I tend to take "safety" with a grain of salt.
However, specifically on TENS units, these are medical devices designed for self-application. Given the litigious nature of this country, I strongly believe that the makers have tried to make them as safe as possible. Add to this that the advice I've been given regarding the use of TENS units in scene play is much more conservative than the makers themselves recommend.
You might want to read Juice by Uncle Abdul. It's an excellent book on electrical play by an electrical engineer. There is also a lengthy section on electrical play in my book High Tech Toys.
