I'm in my 50's, so that may play a part in this perception. A gentleman greets a lady with a kiss to her hand. Now, that means the lady has extended her hand - not to shake hands, but to be held and then lightly kissed. He does not grab her hand so he can kiss it. That's something different, perhaps more modern and flirty. So, as a writer, I guess it depends on your characters and what you mean for the gesture to convey. The attempt to kiss a hand which was extended to shake hands, may be quickly withdrawn showing conflict between the characters. You can play with it.
