As a serious student, student teacher, and eventually black belt instructor of the martial arts for decades, the short answer is no.
I have kicked and been kicked by some of the best. Including professional football players. YES, starting line professionals. No one died in all of the decades of training. All of this was NON-bdsm. A masochist would not enjoy it because the point of the exercise was to mentally block out the pain and continue to be able to engage in combat. The Sadist side of me did not enjoy it because the once a student was properly trained to block out the pain, then there was no pain or reaction. YES, I know that is hard to believe, but I did say properly trained.
Not only did it include kicks, but punches, strikes and (pardon the pun) blows of many types. Think closed fist, open palm-heel, or even the (for the uninitiated) "karate chop". No one ever died, no one ever ended up in the E.R. of the local hospital, no one reported any lasting problems. Yes, the potential for damage is there and injury can happen as with anything. But it is not like you see in the movies.
Just to add, even after decades of such "crotch shots", I am still quite functional (says the female half of the profile) and also have fathered children. So I can testify that those are not issues either.
