No difference. E-stim is the term, not the type of box. The higher end made-for-play boxes (ET312 being the Cadillac) are exceptionally powerful, programmable (often with sound files, creation of which is my specialty) and generally very expensive.

Both TENS and made-for-play boxes can use the same pads and insertables. Sometimes adapters need to be used on the lead-wires since the connections can vary box to box.

I have a small TENS unit that is about the size of a pack of smokes and even has a belt clip. The wires to the pads are very slim and go easily under clothing. It runs on a single 9V battery and packs a WHALLOP if you turn it up.

As someone who bottoms with the violet wand a lot I can say that this device can be used safely on nipples. However, the person using it must know what they are doing, and the sensations won't be anything like the OP mentioned. You also cannot leave the wand on one spot for very long or you can risk electrical burns. In the end the violet wand wouldn't be useful in such a scenario. I'm really just a huge fan of the device, and I know that it can be used on nipples.
