well, sadistic gags, are easiest to shop by diameter honestly. Because it is very easy to go from uncomfortable to hospital visit, heh. To explain, the nature of the way the human jaw works, it is easier to wedge a large diameter ring into position, forcing the jaw uncomfortably open, than it is to get it back out again (well, without pain to clarify). Since everyone can open their mouth to different sizes, just find out what that is, then shop around for ring gags that get there. Keep in mind, you can go for ring, tube (even more uncomfortable because these push at the teeth not behind them), or inflatable. Now the other way to go is measure the internal diameter, for the well, "insertable" purpose. This is because the inner diameter will restrict the outer and unfortunately give you a starting point, good or bad. As for some good suggestions on shops, JT stockroom is a bit limited on ring gags, but high quality, Extreme Restraints a little better on choice. Most likely though if you have an odd diameter, you are gonna have to shop around a bit
