I can get hard on demand - unless I'm very tired - and I can hold it even when I would swear holy oaths that I'm just about to shoot (master says "I don't think so boy" and that's enough) but now the aim is to be able to do it when told to.
Actually it has happened once but that was after a long period of denial and tickling. I was told to roll over and get it out, for a short time I thought that I was peeing on myself! Master had established a fixed penalty for using my hands and I was tempted to take the deal - but he has now withdrawn the offer. Has anyone accomplished this? Are there any tricks I could use?

I've had success with two things.

The first involved someone who normally had a difficult time achieving orgasm, but once sufficiently turned on would go wild when told to come. Eventually she was able to react with less and less buildup beforehand. Starting that sort of training by only being able to orgasm when it's specifically allowed, and that should help build the association.

The second involved hypnosis to have them remember and relive intense orgasms. Note that I strongly recommend a few things if you want to go down this route; that the dom do a fair amount of homework, that you practice with basic trance induction before working on more detailed suggestions, and that you spell out circumstances where the suggestions are null and void. (E.G: When behind the wheel or around non-lifestyle people.) It's pretty safe and effective so long as everyone has their basics down, but you want to make extra sure that the basics have been solidly covered.

Thanks for your reply! I have complete trust in my master. It feels good to know that it's possible. It seems that practice makes perfect, as always.
