From a person who has had surgery while awake, I will never let them do that again to me. Most locals and nerve blocks are done with some from of Lidociane (Maracaine, bupivacaine, ropivacaine, sensorcaine, etc) but there are other drugs too. While these drugs are fairly effective on most people, some people are rather resistant to their affect. I happen to be one of those unlucky people. Anesthesia is a pretty major shock to the body. I can see the advantages of using local only from a medical perspective. The recovery time is much faster and it would reduce medical costs. A person spends 1 to 3 hours in post op requiring skilled care. Eliminating that would be a cost reduction (that you will never see) for the hospital. Doctor's and staff will need to be re-educated on how to talk and act while performing surgery now that the patient can hear and understand everything the doctor and nurses say. "please pass the saw nurse" might not go over well to the patient. I would assume though that a drug like Versed or some other drug to make the patient be more in a twilight sleep would really be needed to keep the patient calm.
